Hernia recurrence is a real challenge for the healthcare system. Surgeons are looking for better surgical techniques to reduce the recurrence rate. Transversus abdominis release (TAR) is a promising new surgical approach based on posterior component separation. Studies have shown that TAR can reduce the recurrence rate even in larger defects and in higher risk patients. As far as the indication for this procedure is concerned, characteristics such as defect size, duration of hernia disease and obesity should be taken into account. Possible contraindications such as wound infection should be ruled out before the operation. The surgical technique is based on PCS with medial transection of the insertion of the transverse abdominal muscle, either unilaterally or bilaterally. Aftercare is important in order to recognize possible complications at an early stage.
