Synovial sarcoma (SS) is often located para-articularly and rarely can be encountered as an intra-articular tumor. The knee is a common site of involvement. It can be seen both before and after skeletal maturity. In this chapter, we present the case of a 10-year-old male patient who had undergone an unplanned resection for an intra-articular soft tissue neoplasm of the knee with a postoperative diagnosis of synovial sarcoma. While surgical margins were positive, no macroscopic residue was evident in imaging at presentation. The conventional course of action was considered as an extra-articular resection with tumor prosthesis reconstruction. An alternative strategy was sought to decrease the significant morbidity associated with extra-articular resection and to facilitate the preservation of the knee joint with margin-modifying adjuvant methods and surgical techniques.
