Özet

The aim of this study was to investigate the effect of intravenous dexamethasone treatment with lung protective ventilation strategia in early term of sepsis. Cannulas were inserted to rabbits ear vein and artery. Ear artery was monitorized for invasive arterial blood pressure and first blood samples were taken for arterial blood gases and malondialdehyde values. Mean arterial pressure (MAP) and heart rate (HR) was recorded every 60 minutes. Laparotomy was performed under sedation. Caecum was tied and punctured. Before the closure of the abdomen, 40 rabbits were randomly divided into 2 groups (n:20). Group I which were not given dexamethasone treatment and, Group II had 3 mg kg-1 dexamethasone administered via intravenous route. Tracheostomy was performed 1 hour after abdomen closure in all groups. All animals were ventilated with lung protective mechanical ventilation strategia for 180 minutes. At the 90 and 180 minutes of the study, blood samples were collected for arterial blood gases and malon dialdehyde determination. At the end of the study, all animals were sacrified for histopathologic examination and determination of wet/dry ratio on lungs. Pathologic lesions were classified ranging from 0-4. MAP (p<0.001), HR (p<0.001) at 60, 120, 180.min, PaO2, (p<0.001, p<0.01), pH (p<0.05, p<0.001) at 90 and 180.min values were the lower in group I than group II. Plasma MDA values and wet/dry ratio were higher in group I than group II (p<0.001). Histopathologic changes were significant in group I than the other group. Dexamethasone administered via intravenous route with lung protective mechanical ventilation had beneficial effects on hemodynamics, PaO2 values, lipid peroxidation and histopathologic changes or wet/dry ratio in early term of intraabdominal sepsis model in rabbits.
