The present report describes the case of a 63-years-old woman presented with extremely pruritic nodule on the perineal skin which progressed gradually over 3 years and did not respond to any topical steroids, antibiotics, and antifungals. The physical examination revealed 5x7 cm. hypertrophic nodüle with erosions, and crusts. The internal malignancies associated with extramammary Paget disease were investigated, but there was no evidence of accompanying visceral malignancy in this patient. Histopathological examination revealed Paget's cells which were positive for Periodic Acid Schiff and Alcian blue stain. Surgical excision was done considering the premalignant potential.
