Periodontal illness perception: Effects of nonsurgical periodontal therapy in smokers and nonsmokers

Background: The authors aimed to evaluate the illness perception and oral health–related quality of life of smoking and nonsmoking patients with periodontitis before and after nonsurgical periodontal treatment (NSPT). Methods: This prospective cohort study included systemically healthy patients with stage III or IV periodontitis. NSPT (ie, oral hygiene instruction and full-mouth scaling and root planing) was provided. Participants completed the Brief Illness Perception Questionnaire and Oral Health Impact Profile-14 questionnaire to assess disease perception and oral health–related quality of life, respectively, at baseline and 21 days posttreatment. Nicotine dependence was assessed by means of the Fagerström Test for Nicotine Dependence. Parametric and nonparametric tests were applied for inter- and intragroup comparisons. The effect of smoking, time, and their interaction was evaluated by applying generalized estimating equations. Results: At baseline, groups differed only in sex distribution, with fewer women among smokers (P = .012). Among smokers, 41% had moderate to high nicotine dependence. Before treatment, nonsmokers had higher scores for personal control (P = .017), understanding (P < .001), and emotional response (P = .038). After treatment, nonsmokers had higher scores for consequences (P < .001), timeline (P < .001), personal control (P = .006), identity (P = .025), concern (P = .027), and understanding (P < .001). Conclusions: NSPT reduced the emotional burden associated with periodontitis and was associated with improvements in illness perception. Smoking status may be associated with variability in treatment-related changes in illness perception. Practical Implications: Smoking status may influence patients’ perceptions of periodontal disease and response to therapy. Clinicians may need to provide additional counseling and support to smokers during periodontal treatment.
