Angiosarcomas are rare malignant soft tissue neoplasms commonly involving the skin and superficial soft tissues. The head, neck, and face are common areas of involvement. The lesions can be protuberant, nodular, and exulcerated. While complete surgical excision is the primary treatment modality, the infiltrative nature of the borders makes it difficult to determine the tumor extent and complicates surgical excision with clean margins. Radiotherapy is an essential adjuvant modality to facilitate surgery. Patients can also benefit from chemotherapy. In this chapter, we present the case of a 26-year-old male patient with fungating and exulcerating angiosarcoma of the head and neck region. The patient was treated with wide resection following concurrent chemoradiotherapy and soft tissue reconstruction with a pedicled trapezius flap.
