An unusually long and thick paracondylar process (PCP) was observed in a dry human skull during a routine laboratory session. It was located postero-lateral to the occipital condyle and medial to the mastoid process, projecting downwards toward the lateral end of the transverse process of the atlas. It was 15 mm long and 31 mm in circumference at its iddle and a smooth articular facet was present on the tip. Large PCPs may lead to the clinical symptoms such as headache, restricted neck movements and osseous torticollis. Awareness of this variation and its morphologic character is of clinical importance especially for radiologists, surgeons and physiatrists.
